Institutional Info
All information that should be standard on all applications from HMS/HSDM is available online at the SPA Institutional Info page. Notable requirements are described below:
- Box 5: Only use the HMS DUNS # 047006379. Double check that you have entered this correctly on the 424 Face Page as it will feed through on subsequent forms.
- Box 5: Applicant Information asks for the applicant institution, i.e., President and Fellows of Harvard College. The PI's name and contact information should not be entered until Box 15.
- Box 14: Make sure that the Congressional District is entered as MA-008. Simply entering 8 is considered incorrect.
