5. Adobe Acrobat Reader may open the document for you as you select it, but if not, open the application pages from within Adobe Acrobat Reader
   
6. To print a blank form, simply print out the application as is.
7. To enter your personal identification information electronically, you may enter it directly into the downloaded form, and then print the form for submission. If you wish to leave a field blank, skip it, and fill that field out by hand after printing.
