F. List of Documents to Be Submitted to Office for Faculty Services with Each Term Appointment Recommendation

1.
Form for Initiation of First Term Appointments and Promotions.

2. Affirmative Action Report for Term Appointments, including initial plan for the search if this is the first appointment in Harvard Medical School.

3. Department head's letter, including the search committee report, if this is a first appointment in HMS on a full-time ladder.

4. Letter from the executive committee of the department if there are any dissents or abstentions; if none, signature of the chairperson or the secretary of the executive committee on the Form for Initiation of First Term Appointments and Promotions is sufficient.

5. Information on previous search if the first appointment was as instructor within the previous six years.

6. Letters of evaluation.

7. Curriculum vitae, including self-report of research, teaching, or clinical activities (as appropriate) and bibliography.

8. Two copies of two best publications (or scholarly communications, as determined by ladder) from each candidate recommended for the rank of assistant professor.

9. Two copies of five best publications (or scholarly communications, as determined by ladder) from each candidate recommended for the rank of associate professor.

10. New Hire Form if this is a first appointment, or one after break in service of more than two years.
