Program in Palliative Care Education and Practice
Application and Admissions Information
Preference for this two-session (Spring and Fall) course will be given to those applicants who submit the completed Application Form along with the required documents. Selection of participants is based on palliative care experience, participant diversity, teaching or research experience, project potential, and institutional commitment. The application deadline has been extended to January 30, 2008. Applications will not be accepted for a single session.

Preference will be given to applications received by January 30th, 2008. Incomplete applications will not be considered.
Along with your registration form, please include the following items:
• A three-page narrative statement about:
1. Your current or planned activities in palliative care, including a description of your role as an educator and/or leader.
2. Your goals for taking this course and how you expect to use this course in your work.
3. A decription from your Dean, Department Chair, Institutional CEO, or a similar institutional leader indicating support for your project and your availability to participate in this course.
• A curriculum vitae that describes your teaching and research responsibilities for the past five years.
